Make sure your anti virus is not blocking the trainer.
Just played the game here with PLITCH and everything was working fine.

I turned on one-hit kills, but they don’t work when I'm playing. I can only knock about two-thirds of my blood out. Why?

I have same situation, I clicked prepare and nothing happened, tried turned off all anti virus including Win10's, tried run plitch as administrator, tired redownload game, tried to use different version of game, nothing happened.
